For Successful Candidate at Grade 5 Scholarship Examination

The Rev Fr H S David Foundation (FHSDF) recognizes academically outstanding students of Jaffna. The mission of the program is to give opportunity to deserving students from the underprivileged society. The exceptional and highly motivated students who meet the standards required by the Foundation will be eligible to apply for the scholarship for the middle grades of 6th, 7th and 8th Grade standards.

GRADE V - SCHOLARSHIP APPLICATION 2014

Students should have sat and obtained marks and should have above the cut off mark of the District.

WHAT DOES THE SCHOLARSHIP OFFER?

School fees for subsequent three progressive years of the selected Student

Set stipend to cover text books, stationery and miscellaneous expenses

Continuous monitoring and psychological support

WHO QUALIFIES TO APPLY?

Students who meet the following criteria may apply:

   1. Extraordinary performance in grade V scholarship exam 2013 (BOYS & GIRLS)

   2. Eleven years or younger at the year of application

   3. Attaining good performance in School activities (sports and extra curricular activities)

   4. Most suitable for Underprivileged children with financial need

WHAT ARE THE STEPS IN THE SELECTION PROCESS?

Students who meet the following criteria may apply:

   1. Applicants should meet the set minimum criteria

   2. Eligible students will face an assessment interview

   3. Qualifying student families will also face for an interviews

   4. Qualifying student be checked of his/her credentials

Successful candidates will be included in the Student Sponsorship Programme in the year 2013 for grade VI in 2014 onwards.

IF YOU QUALIFY PLEASE COLLECT & SUBMIT APPLICATION FORM:

QUALIFYING STUDENTS NEED TO DO THE FOLLOWING:

Attach copies of the following items along with the completed form:

        i. Grade 5 scholarship exam results of 2013

        ii. Copy of birth certificate

      (In a case of orphans or vulnerable children, a letter from the caregiver’s organisation / social         worker must be attached stating the reason/s)

        iii. Copies of the proof of income of both parents/legal guardians

     (If the parents/legal guardians/care-givers are unemployed, an affidavit or certificate from GS         stating the same must be attached)

        iv. Any other documents required by the selectors

IMPORTANT NOTICE TO PARENTS / GUARDIANS/CAREGIVERS

Submitting an application form does not guarantee the awarding of the scholarship. Applicants need to qualify and complete the selection process before being considered for the scholarship. The decision to award the scholarship rests solely with the Student Sponsorship Programme of Fr H S David Foundation.

Only applicants who meet the minimum application criteria will be processed. It is important that parents/guardians/caregivers provide us with the correct and accurate details.

Incomplete applications that do not meet the criteria will be rejected

The mission of the program is to give opportunity to deserving students from the underprivileged society. The exceptional and highly motivated students who meet the standards required by the Foundation will be eligible to apply for the scholarship for the secondary grades of 11th, and 12th Grade standards.

GRADE GCE (O/L) - SCHOLARSHIP APPLICATION 2014

Students should have sat and obtained marks and should have above the cut off mark of the District.

WHAT DOES THE SCHOLARSHIP OFFER?

School fees for subsequent progressive academic years GCE (A/L) of the selected Student

Set stipend to cover text books, stationery and miscellaneous expenses

Continuous monitoring and psychological support

WHO QUALIFIES TO APPLY?

Students who meet the following criteria may apply:

   1. Extraordinary performance in GCE (O/L) exam 2012 (BOYS)

   2. Should be the first attempt of GCE (O/L) at the year of application

   3. Attaining good performance at St Patrick’s College in all activities

   4. Most suitable for Underprivileged children with financial need

WHAT ARE THE STEPS IN THE SELECTION PROCESS?

Students who meet the following criteria may apply:

   1. 1. Applicants should meet the set minimum criteria

   2. 2. Eligible students will face an assessing interview

   3. 3. Qualifying student families will also face for an interviews

   4. 4. Qualifying student be checked of his/her credentials

Successful candidates will be awarded the Student Sponsorship Programme in the year 2013 for GCE (A/L) in 2014 onwards for progressive academic years till he/she attempts the first time.
